Is there any fee to apply for a CPR?

If so, how much is it?
And where do I apply?

Can I go in person, or does my sponsor have to do it for me?

I am not presently working in Bahrain but I have worked there before for 4 years. Before I left the country, my CPR card has just been newly renewed and it will expire next year (2015). If in case I will be able to get a new job there, is there a need for me to get a new CPR card or just a change in the employer? Thanking everyone in advance for any input you can give.

You will only have to change the employer if your card has not expired yet, you rather renew the card at the same time.
